  2. Please limit to 15 words. Your credentials will be used in the promotion of this program. Only include what is relevant to this program.

  3. Where/how would you like to teach your class?*
  4. If APRC facilities are not permitted to open this season, how would you like to teach your class?
  5. Please include answers to the following questions and limit to two sentences: 
    What knowledge and skills will students receive for the course fee? 
    What should participants bring to class? 
    Any additional information that is needed from participants besides basic contact information; e.g. proper clothing to wear, water bottle, pen and paper, etc.?

  7. APR limits registration to this number. Final number depends on facility size.

  8. Facility Requirements

    These options are available only in APR buildings, or in park areas adjacent to the Nature Center and Senior Center.

    • By the end of this program, the participants will be able to explain the benefits of different kinds of bird feeders.

  16. APR instructors are required to adhere to and monitor compliance with current OHA public health guidelines related to the type of recreation activity delivered. Please initial.

  17. By submitting this form you understand that this proposal is subject to approval by APR staff. Instructors will be responsible for designing curriculum, activities and providing extra supplies not included in supply list. Course approval may also require instructors to meet requirements for affiliation with APR and the City of Ashland. Please initial here.
