Use this permit application to apply for a Haunted House Permit. Haunted houses and similar type temporary installations are any activity that uses a maze concept inside a structure. Requests should be made at least five working days prior to the event. Larger events may require longer processing times.
At the end of this application is a place to upload an event site plan. Please have this ready before applying for the permit. Please include the following information on your site plan:

The site plan needs to show a general overview of the area and proximity of your event to adjacent buildings, public ways and vegetation. Please include the following information on your site plan: Event Name and Date, Property lines and dimensions in feet, Adjacent Buildings and distance in feet, tent and/or booth locations and dimensions, generators, cooking appliances, heating appliances, barricades, exits, and fire extinguishers.
After selecting submit, you will receive an email confirming that your application has been received. Please note that until a permit is signed by you and an Ashland Fire and Rescue representative, the permit has not been issued.
If you were unable to upload a site plan of your event with you permit application, you will need to email or fax one to Ashland Fire and Rescue. While it is not required to be drawn to scale, dimensions will need to be shown on the drawing. Items that need to be included on the site plan will be listed on your confirmation email.
