Use this permit application to apply for a permit to use Open Flames or Pyrotechnics in front of an audience. Open Flames and Pyrotechnics are prohibited in the city of Ashland unless a permit has been obtained from Ashland Fire and Rescue. Requests should be made at least five working days prior to the event. Larger events may require longer processing times.
At the end of this application will be a place to upload an Event site plan and a List of open flame or pyrotechnics to be used. Please be specific in the types of flames and/or pyrotechnics as to type, number of and when they will be used. Please have this ready before applying for the permit. Please include the following information on your site plan:

If a single event, please state start time & end time of event (hh:mm am/pm). If there are multiple event dates and times, please include the entire schedule here or upload as an additional document at bottom of form.
The site plan needs to show a general overview of the area of your performance. Label all distances/dimensions in feet. Please show performance area boundaries, audience location(s) and fire extinguisher locations. If outdoors, also include the following information: the proximity of your event to adjacent buildings, public ways, tent and/or booth locations (and dimensions of each), and distances to property lines.
Provide a list with a detailed description of the open flames and pyrotechnics to be used, where they will be used and for how long they will be used.

After selecting submit, you will receive an email confirming that your application has been received. Please note that until a permit is signed by you and an Ashland Fire and Rescue representative, the permit has not been issued.
